Forms for Camp Squanto are here.
Online forms have a entry screen for you to type in your name, etc. before printing the form. The data is retained in your browser's "cookie" file so you'll only have to enter it once. You still have to print and submit the form yourself, but we're working on that.
Blank forms are just that: you print them first, fill them out with a pen, and send them in.
